Cancellation & No-Show Policy
We require 24 hours’ notice for cancellation of appointments. A no show fee of $40 applies to regular visits, and $60 applies to physical’s or surgical procedure appointments. You may be required to pay all fees due prior to receiving medical care from your physician.
Abuse Policy
We have a zero-tolerance policy for verbal abuse and aggression directed towards staff, be it in person, or during a telephone conversation. Anyone who violates this policy will need to seek care elsewhere. You will be asked to leave the clinic and can be denied medical services.
Confidentiality Policy & Procedure
Our staff is certified to protect your information by abiding by the requirements under the Personal Health Information Act (PHIA). You have the right to your personal health information. All patients must provide written and witnessed authorization to have your medical information released to anyone without you being present.
